and then click in the right side of the window.&nbsp; The buttons at the bottom of the window will change to reflect that you are now in the Case section.<\/p>\n\n\n\n<p>03.\u00a0 If you want to edit data-recalc-dims=\"1\" \/><\/figure>\n\n\n\n<p>05.\u00a0 Starting at the top of the PERSONAL tab, the Case Number field will be automatically assigned by the program and will be a unique number.\u00a0 First enter a DESCRIPTION for the case.\u00a0 What you enter here may depend on your reason for creating different cases.\u00a0 Some examples are:\u00a0 Headaches, or Medicare, or November, or Dr. Jones, etc.<\/p>\n\n\n\n<p>06.\u00a0 If this patient does not have insurance and is going to pay cash then place a check-mark in the \u201cCASH CASE\u201d box.\u00a0 Please note, \u201cCASH CASE\u201d patients should not have an insurance entered on the Policy 1, 2, or 3 tabs.\u00a0 Also note, if the \u201cCASH CASE\u201d box checked and that \u201cPRINT PATIENT STATEMENT\u201d box is not checked the patient\/guarantor will not receive a statement.\u00a0 If the \u201cPRINT PATIENT STATEMENT\u201d box is checked the program will allow you to print statements to the patient or guarantor.<\/p>\n\n\n\n<p>07.\u00a0 If you enter a procedure for this case that has Global Surgical Procedure as the procedure type, the GLOBAL \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 COVERAGE UNIT field will default a date according to the Global Period entered in the procedure code&#8217;s record.<\/p>\n\n\n\n<p>08.\u00a0 The GUARANTOR field will default to the patient\u2019s Chart Number, which is fine if they\u2019re self-insured, but should be populated by the Guarantor\u2019s Chart Number if not.<\/p>\n\n\n\n<p>09.&nbsp; If known, select a MARITAL STATUS and STUDENT\nSTATUS which correspond to Box 8 on the CMS-1500 form.<\/p>\n\n\n\n<p>10.\u00a0 You can enter information in the \u201cEmployment\u201d section if known, but this is typically only needed on Workers Comp type claims.<\/p>\n\n\n\n<p>11.\u00a0 Next click on the ACCOUNT tab.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" loading=\"lazy\" width=\"630\" height=\"582\" data-recalc-dims=\"1\" \/><\/figure>\n\n\n\n<p>12.\u00a0 In the top section of this tab you can specify Provider(s), Referral Source, Attorney, and Facility for the case.\u00a0 The ASSIGNED PROVIDER is the provider in your office whom performed the health services or procedures on the \u00a0\u00a0\u00a0\u00a0 patient.\u00a0 <\/p>\n\n\n\n<p>13.\u00a0 In the next section you can specify a CASE BILLING CODE which is a user-defined field for whatever purposes you may need.\u00a0 The code is limited to 1 to 2 characters (A-Z, 0-9).\u00a0 For example, you could differentiate between \u00a0\u00a0\u00a0\u00a0 Workers Comp (WC), and standard Claims (SC), or split billing responsibility between two billers (B1, B2\u2026), etc.<\/p>\n\n\n\n<p>14.\u00a0 The PRICE CODE field determines which charge amount specified in the Procedure Code gets used.<\/p>\n\n\n\n<p>15.\u00a0 The OTHER ARRANGEMENTS field is another user-defined field for whatever purposes you may need.\u00a0 This field\u00a0 allows for a 4 character code (A-Z, 0-9).<\/p>\n\n\n\n<p>16.\u00a0 In the last section of this tab you can specify an AUTHORIZATION NUMBER (assigned by the insurance), the LAST VISIT DATE which is auto-populated by the last visit entered in Transaction Entry (but you can make changes if   needed), the AUTHORIZED NUMBER OF VISITS, and ID, and the LAST VISIT NUMBER.<\/p>\n\n\n\n<p>17.\u00a0 Next click on the DIAGNOSIS tab.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" loading=\"lazy\" width=\"659\" height=\"425\" data-recalc-dims=\"1\" \/><\/figure>\n\n\n\n<p>18.\u00a0 In the top section of this tab you can specify a PRINCIPAL DIAGNOSIS and DEFAULT DIAGNOSIS 2, 3, and 4.\u00a0 It is not required to enter diagnoses here, but if you do they will automatically populate when entering charge procedures in Transaction Entry (which will be discussed in a later manual).\u00a0 The POA column can be checked to signify the diagnosis was \u201cPresent on Admission\u201d.<\/p>\n\n\n\n<p>19.\u00a0 The EDI REPORT section is for attachments for claims.\u00a0 See the help article \u201cCase \u2013 Diagnosis\u201d for more information on what code values to enter.<\/p>\n\n\n\n<p>20.\u00a0 Next click on the POLICY 1 tab.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" loading=\"lazy\" data-recalc-dims=\"1\" \/><\/figure>\n\n\n\n<p>21.\u00a0 This tab is where the patient\u2019s primary insurance and other information is entered.\u00a0 If the patient pays cash and  does not have insurance, do not enter anything here.\u00a0 Otherwise enter the INSURANCE 1 code, or select it via the \u00a0\u00a0\u00a0\u00a0 drop-down arrow, or lookup (Magnifying Glass).\u00a0 Next select the POLICY HOLDER 1, either the patient or the \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 guarantor.\u00a0 If not \u201cSelf\u201d select the RELATIONSHIP TO INSURED.\u00a0 Enter the POLICY NUMBER, and if applicable the \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 GROUP NUMBER and GROUP NAME.\u00a0 The Policy Dates are optional, but could be entered if known.\u00a0 The CLAIM \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 NUMBER field is for electronic claims sent in the ANSI format.\u00a0 It is used on property\/casualty\/auto claims.\u00a0 This \u00a0\u00a0\u00a0\u00a0\u00a0\u00a0 number is assigned by the property and casualty payer.<\/p>\n\n\n\n<p>22.\u00a0 In the next section place a check-mark in the ASSIGNMENT OF BENEFITS\/ACCEPT ASSIGNMENT field if the practice is to receive payment from the insurance payer.\u00a0 Otherwise payment will go to the patient \/ guarantor.<\/p>\n\n\n\n<p>23.\u00a0 Click the CAPITATED PLAN check box if the patient&#8217;s plan is capitated; leave it blank if not.\u00a0 If this field is checked,\u00a0 enter 100 in the Insurance Coverage Percents by Service Classification A field.\u00a0 On the secondary insurance tab, this option becomes Crossover Claim. <\/p>\n\n\n\n<p>24.\u00a0 The DEDUCTIBLE MET field is used to indicate whether the patient has met his or her annual deductible.\u00a0 You can enter the ANNUAL DEDUCTIBLE and COPAYMENT AMOUNT in those fields.<\/p>\n\n\n\n<p>25.\u00a0 In the TREATMENT AUTHORIZATION field you can enter the treatment authorization code from the insurance \u00a0\u00a0\u00a0 company.\u00a0 This is typically only used with UB-04 claims.<\/p>\n\n\n\n<p>26.\u00a0 If applicable enter the DOCUMENT CONTROL NUMBER (original control number) if processing a replacement claim or voided claim.<\/p>\n\n\n\n<p>27.\u00a0 See the \u201cInsurance Coverage Percents by Service Classification\u201d article in the help files for more information on the last section of this tab.<\/p>\n\n\n\n<p>28.\u00a0 If the patient has a secondary insurance click on the POLICY 2 tab to enter that information.\u00a0 Also, if the patient has a tertiary insurance, click on the POLICY 3 tab to enter that information.\u00a0 Both of those tabs are filled out similarly to\u00a0the POLICY 1 tab.<\/p>\n\n\n\n<p>29.\u00a0 Next click on the CONDITION tab.<\/p>\n\n\n\n<figure &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; Symptoms), and 24c (Emergency).<\/p>\n\n\n\n<p>31.&nbsp; The next section relates to Boxes 10 (Auto\nAccident), 16 (Unable to Work), 18 (Hospitalization), and other &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; information for Workers Comp Claims,\netc.&nbsp; The bottom of this section is for\nCondition Codes used with UB-04 &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp; claims.<\/p>\n\n\n\n<p>32.\u00a0 Next click on the MISCELLANEOUS tab.<\/p>\n\n\n\n<figure class=\"wp-block-image\"><img decoding=\"async\" loading=\"lazy\" amount.<\/p>\n\n\n\n<p>32.&nbsp; The LOCAL USE A field is typically used to\npopulate Box 10d, while the LOCAL USE B field is used to populate Box 19.<\/p>\n\n\n\n<p>33.\u00a0 The case INDICATOR code is an up to five-character user-defined alphanumeric code that can be used to divide the practice into groups for sorting diagnoses, etc.\u00a0 The Case Indicator field in the Create Claims window is one of the possible filters to identify claims.\u00a0 This field is used strictly as a practice management tool. <\/p>\n\n\n\n<p>34.&nbsp; REFERRAL DATE and PRESCRIPTION DATE can be\nentered if known.<\/p>\n\n\n\n<p>35.&nbsp; If a PRIOR AUTH NUMBER was obtained it can be\nentered here to populate Box 23 on the claim form.<\/p>\n\n\n\n<p>36.\u00a0 The EXTRA 1, 2, 3, and 4 fields provided in this window are user-defined fields which can be used for special entries required by insurance carriers.<\/p>\n\n\n\n<p>37.\u00a0 If the Primary Care Provider is not a physician who practices in your office he or she can be specified in the OUTSIDE PRIMARY CARE PROVIDER field, and the DATE LAST SEEN by that provider can be entered here.<br><br>38.\u00a0 Next click on the MEDICAID AND TRICARE
